Referral Form - Link

How to refer - Referrals are preferred by fax. Please fill in the patient's and the referring physician's contact information as well as all pertinent medical information. Please note the pre-test instructions and pass this information on to your patients. Patients will be contacted by Peace Country Lung Lab to schedule the appointment.

Lab Access - An external ramp is available for medical professionals to access the lab with a stretcher or wheelchair.

Pediatrics - Minimum age for testing = 7 years of age.

Results - Interpreted results will be provided within 24 to 48 hours after completion of the test. Results will be faxed to the referring physician.

Pre-Test Instructions

If possible, patients should avoid taking:

Short-acting bronchodilators (e.g. Berotec Ventolin, Atrovent, Airomir, Symbicort) for at least 4 hours prior to the start of your test. Long-acting bronchodilators (e.g. Serevent, Oxeze, Advair or Symbicort) for at least 12 hours prior to the start of your test.

**You can continue to take all other prescribed medication including anti-inflammatory medications such as Beclovent, Qvar, Vanceril, Bronalide, Pulmicort, Prednisone and Flovent.**
